Privacy Policy & HIPAA Notice Summary

Who we are: REV for Men, LLC (“we,” “us,” “our”) operates revformen.com and provides men’s health services in Florida.

1) What this notice covers

This summary explains:

  • How we collect and use information on our website; and

  • How we use and disclose Protected Health Information (PHI) if you become a patient, consistent with HIPAA. 

If you are a patient, we are required by law to protect the privacy of your PHI and to provide you with this notice of our privacy practices. 

2) Information we collect

A. Website / consumer information (non-medical):

May include contact details you submit (name, email, phone), device and usage data (IP address, pages viewed), cookies, and similar technologies.

B. Health information (PHI) (if you request or receive care):

May include symptoms, diagnoses, medications, lab results, treatment plans, payment/insurance information, communications with our clinical team, and related records. HIPAA protections apply to PHI when we act as a covered healthcare provider. 

3) How we use your information

Website information may be used to:

  • Respond to inquiries and provide requested services

  • Operate, secure, and improve the website

  • Send appointment or service messages you request

  • Conduct analytics and measure performance (with service providers)

PHI may be used and disclosed as permitted by HIPAA, including for:

  • Treatment (e.g., coordinating care with providers)

  • Payment (e.g., billing, collections)

  • Healthcare operations (e.g., quality improvement, auditing, training) 

4) When we may share information

We may share information with:

  • Vendors/service providers who help us run our operations (e.g., scheduling, hosting, EHR, payment processors). Where required, we use appropriate agreements (including Business Associate Agreements for HIPAA vendors).

  • Labs, pharmacies, and other partners as needed for treatment and care coordination.

  • Legal and regulatory authorities when required by law (e.g., subpoenas, licensing, public health reporting, lawful requests). 

We do not sell your PHI.

5) Your HIPAA privacy rights (for PHI)

If HIPAA applies to your information, you have rights including:

  • Get a copy of your medical record (access)

  • Ask us to correct your record (amendment)

  • Request a list of certain disclosures (accounting)

  • Request restrictions on certain uses/disclosures (we may not be required to agree, with limited exceptions)

  • Request confidential communications (e.g., contact method)

  • Get a paper/electronic copy of this notice 

6) Florida confidentiality and breach notifications

Florida law includes additional protections around the confidentiality and handling of medical records by healthcare practitioners. 

If a security incident involves unsecured PHI, HIPAA may require patient notification. 

If a security incident involves certain personal information of Florida residents, Florida’s breach notification law may require notices—including notice to the Florida Department of Legal Affairs for breaches affecting 500+ Florida residents—generally within 30 days of determination.

7) Cookies, analytics, and advertising

We may use cookies and similar tools to operate the site, remember preferences, prevent fraud, and understand traffic. You can control cookies through your browser settings and, where offered, our cookie controls.

8) Communications (email/SMS)

If you provide your phone number or email, we may send you messages related to your inquiry, appointments, care, billing, or requested information. Message/data rates may apply for SMS. You can opt out of non-essential marketing messages at any time.

9) Children’s privacy

Our services are intended for adults. We do not knowingly collect personal information from children under 13 without parental consent.

10) Security

We use reasonable administrative, technical, and physical safeguards designed to protect information. No system can be guaranteed 100% secure.

11) Changes to this notice

We may update this notice from time to time. The current version will be posted on our website and will include an updated effective date. HIPAA requires us to follow the terms of the notice currently in effect.
